新型コロナウイルス感染症への感染が判明した際のご連絡について（学生及び教職員の皆様へ）【2023年5月8日以降】

 

◆“学生・教員”専用入力フォーム／For Undergraduate Student, Graduate Student、Faculty Member (Full time/Part time)　

※出校停止期間の授業配慮書類（新型コロナウイルス感染症治癒報告書）の作成根拠となりますので、罹患した場合は、所属学部・研究科のGoogleフォームに必ず入力してください。
※授業配慮を受けるためには、新型コロナウイルス感染症にり患したことを示す書類等（PCR検査・抗原検査等の陽性判定結果や医療機関が発行した診療明細など、新型コロナウイルス感染症にり患したことが分かるもの）が必要です。
※治癒後、罹患したことを証明できるものをお持ちの上、各キャンパスにある保健センターにお越しください。治癒報告書を作成します。
